Consent form
Indicating the people with whom ILCT is permitted to exchange information. For example, a doctor, relatives, other health professionals, etc.
Service agreement
A contract between ILCT and the client to outline services, expectations, and time frames for the nominated service(s).
ILCT staff will not be able to provide services without a completed service agreement.
